For this reason, our fertility department places equal importance on evaluating both male and female partners rather than assuming that infertility is primarily a female issue.
Low or absent sperm count can have several possible causes, ranging from hormonal or medical conditions and previous infections or surgeries to problems affecting sperm production or the passage of sperm. In some men, no clear cause is identified. A detailed evaluation helps determine the most appropriate treatment and whether the couple may benefit from natural conception, medication, IUI, IVF, or specialized sperm retrieval techniques.
The evaluation generally begins with a detailed medical, reproductive, and lifestyle history. Where a specific medical or hormonal condition is identified, appropriate treatment may be recommended. Some hormonal conditions can affect sperm production, and treating the underlying disorder may improve reproductive function in selected patients.
However, lifestyle changes or medication are not suitable as the sole treatment for every case of severe male-factor infertility. The treatment plan depends on the severity and cause of the sperm abnormality and the fertility status of the female partner.
Intrauterine insemination (IUI) may be considered when the sperm count and quality are sufficient for the procedure and other fertility factors are favorable. During IUI, sperm is processed in the laboratory and the more suitable sperm are concentrated before being placed directly into the uterus around the time of ovulation.
IUI may be appropriate in selected cases of mild male-factor infertility. However, when sperm count or motility is severely reduced, IUI may offer limited benefit. In such circumstances, the specialist may recommend IVF or another assisted reproductive technique instead.
The decision is based on the complete fertility assessment, including the woman's age and ovarian reserve, fallopian tube condition, duration of infertility, and previous treatment history.
For more significant male-factor infertility, in-vitro fertilization (IVF) may provide a more effective treatment pathway. IVF involves collecting eggs from the woman and fertilizing them with sperm in a laboratory before transferring a suitable embryo into the uterus.
When sperm numbers or movement are severely reduced, a specialized technique called intracytoplasmic sperm injection (ICSI) may be considered. ICSI involves injecting a single sperm directly into an egg to assist fertilization.
The decision to use ICSI depends on the individual clinical circumstances and laboratory findings. It can be particularly useful when conventional fertilization is unlikely to be successful because of significant sperm abnormalities or when very few sperm are available.
In cases of very low or absent sperm count, sperm may sometimes be obtained directly from the reproductive tract or testicular tissue using a specialized sperm retrieval procedure. The exact technique depends on why sperm are absent from the semen and where viable sperm are expected to be found.
Retrieved sperm may potentially be used for ICSI when suitable sperm are available. These procedures require careful assessment and should only be recommended when there is a reasonable clinical indication.
